Join the Cavity Busters Club
At Care Orthodontics, our top priority is your healthy smile. We constantly take good care of your smile and we want you to maintain a healthy mile. With this goal in mind, we created this Cavity Buster' Club.
The program is simple . . . here's how it works:
- Visit your general dentist for a regular checkup and
cleaning every six months. Print out this form Cavity Clearance Form.pdf and take it with you to your family dentist. Your dentist will fill out this report.
- Return this completed form to our office to redeem tokens.
These tokens, along with others tokens from the Smile Rewards Program can be redeemed to earn great prizes!
